Maine PCMH Pilot Phase 2 Expansion & MaineCare Health Homes Initiative

Maine Quality Counts will be hosting a Provider Lunch & Learn webinar on Tues, Feb 7 at 12N to provide more information on plans for the Maine PCMH Pilot Phase 2 Expansion and the MaineCare Health Homes initiative.

The Maine Patient Centered Medical Home (PCMH) Pilot is an exciting initiative to transform primary care delivery and payment to deliver more high-value, patient-centered care.  The PCMH Pilot currently includes 26 practices from around the state, and Pilot Conveners have recently announced plans to expand the Pilot to include an additional 20 adult practices in January 2013, with an application process to begin by Feb 1, 2012 (for more information, please visit www.mainequalitycounts.org).  In addition, MaineCare, Maine's Medicaid program will be launching a "Health Homes" program that will provide an opportunity for additional practices to be recognized and receive additional support from MaineCare for providing medical home services and intensive care coordination for high need MaineCare members.
